Completing the TRP Plan

Downloading the Plan Format

The TRP Task Force has approved the use of a new, simplified plan format for all employers in the program. The new format will make it easier to complete and revise the plan from year to year. If you are currently using the old, multi-page format, please convert to the new format when your next plan is due to Maricopa County.

To download a copy of the plan format, follow these steps:

Step 1.

Go to “Plan Formats” below and download the appropriate PLAN FORMAT. There are two Microsoft WORD formats available and one .PDF format which requires Acrobat Reader to open. The first format should be used if you plan to use your computer to compete the plan. This format has spaces that will expand as you key in the information. Use the second WORD format if you are going to handwrite the information in the lines provided on the form. The third format is a .pdf file, which may be opened with Acrobat Reader. Please note that if you do not have Microsoft Word software, you will need to use the .PDF plan format. Other word processing programs will corrupt the file.

Open the appropriate plan format.

Step 2.

Go to “file” on the menu bar, and click “Save as.”

Step 3.

Name the document. Save the file in the appropriate directory on your hard drive or disk. Click “OK.”

Step 4.

Employers with more than one mandated site must also
download the “Multiple Sites” form. There are two WORD formats for this form as well, one for completing on your computer and one if you are going to handwrite the information. Follow the information in steps 2 and 3 to download the multiple sites form.

If you have more than eight (8) additional sites, download the multiple sites form as many times as needed, giving each form a new name (Sites 1 – 8; Sites 9 – 16, etc.)

  PLAN FORMATS

NOTE: If you are using either of the handwritten formats, you may not need to save the form to your computer’s hard drive or a disk. Once the plan is open, you can print the form and complete it using blue or black ink. Please write legibly!

If you have problems downloading the document or the opened document is unintelligible, e-mail rideshareinfo@ValleyMetro.org, contact your Valley Metro Rideshare Representative or call Valley Metro Rideshare at 602.262.7433 for assistance.
